There are four ways to process a win loss document request: fax, mail, in-person at the Club M® counter or online.
ONLINE
Access Win/Loss Statements and W-2G Winnings Documentation with a Club M online account. Only Club M members with an online account can access statements.

Download and complete Win/Loss W-2G Winnings form and fax to (952) 496-7160.
BY MAIL
Download and complete Win/Loss W-2G Winnings form and mail to:

IN PERSON
Download and complete Win/Loss W-2G Winnings form and bring to the Club M counter.
To avoid processing delays make sure the form is completed. Your Social Security number is required. Request forms are processed within five business days. THIRD PARTY
If you are a legal guardian or have power of attorney, and are requesting W-2Gs or win/loss statements on behalf of another person, please download and complete the request form below.
