W2G Request Form

To receive a report of your W2G / 1099 Misc Prize Form please complete the attached form (one for each person) and return it to Mystic Lake Casino Hotel in one of the following ways:

To avoid processing delays make sure the form is completed. Your Social Security Number is required. Request forms are processed within 5 business days. W2G documents are mailed to the address on the request form. If you prefer the documents be faxed, please indicate your fax number on the request form.
